As of February 7, all Cornerstone clinics moved to a new, improved electronic health record with an easy-to-use patient portal called myWakeHealth.
The myWakeHealth portal replaces MyCornerstone. On May 1, 2017, the MyCornerstone portal will be discontinued.
When you go for your clinic visits, your health information is now updated in myWakeHealth. Only your test results from the past two years will appear in myWakeHealth; however, your Cornerstone and Wake Forest Baptist doctors will always have access to all of your historical information.
If you wish to have copies of any information in your MyCornerstone account, you can email or print it until May 1, 2017. Log in to your MyCornerstone account and follow the instructions in the screenshot below.
All of your Cornerstone medical records are archived with Health Information Management. If you want to get a copy of your medical records, download and complete a medical release form. Fax all medical record requests to 336-802-2728. For medical record questions, call 336-802-2727.
