Each member of the staff at High Point Family Practice looks forward to providing you
and your family with quality medical care in a friendly, comfortable environment.
The information presented here is provided to help familiarize you with our practice. We are
pleased that you have chosen High Point Family Practice as your primary care provider. Our
health care team is committed to making your experience with our practice as pleasant as possible.
Family Medicine
The purpose of Family Practice is to provide comprehensive medical services to all members of
the family. Each of our physicians, physician assistants, and nurse practitioners has passed their
respective certification examination and regularly attends conferences and meetings to remain
up-to-date on the most current treatments.
Family medicine providers manage pediatric, adolescent, and adult medicine. We provide newborn and infant
care, gynecologic examinations, minor office surgery (including repair of lacerations, sprains and
minor fractures, and treatment of skin lesions), and treatment of most acute and chronic illnesses.
Occasionally, you or a member of your family may need referral to other specialists or to
community resources for specialized evaluation or treatment. If that need arises, we can help you select
that specialist or resource most appropriate for your needs. In those situations, we can continue to
work along with you and your family to aid in your total care and to help you understand the
often confusing language of the medical profession.
Our Objective
As family medicine providers, we believe that the prevention of disease and the maintenance of good
health are the most important goals we can achieve for our patients. These goals are most
likely to be reached when our patients are actively involved in this process as part of our
health care team.
New Patients
You may request to see a specific healthcare provider or we can choose one for you.. Our
health care providers and most of our patients prefer to have the same health care provider for all of the members
of their family who are treated in our office.
Whenever possible, we schedule patients to see
their health care provider. If he or she is not available, another provider will be available to see you.
In keeping with our emphasis on prevention, we will ask all new patients to schedule a complete
physical examination, allowing us to address health maintenance issues and establish a baseline
on your health that will assist your primary care provider in effectively treating any illnesses that may
arise.
Appointments
Our office is open from 8:00 a.m. until 8:00 p.m. Monday through Thursday, 8:00 a.m. until 5:00 p.m.
on Friday, and for urgent needs, we are open 8:00 a.m. until 12:00 noon on Saturday and 1:00 p.m. until 4:00 p.m. on Sunday.. Prescription refills, scheduling of routine
appointments and other non-urgent questions are handled each weekday between the hours of 8:00 a.m. and
5:00 p.m.
We generally see patients by appointment. When you call for an appointment, please explain the
nature of your problem. This will allow us to schedule your appointment promptly and efficiently.
If you have an urgent problem but do not have an appointment please call our office prior to coming
in. Our triage nurse can make sure you are seen the same day when necessary. This is especially true
when calling earlier in the day. We keep a high volume of appointments open for same day visits.
If you drop by without an appointment you may wait longer to be seen than if you call in first.
To help us stay on schedule, and as a courtesy to our other patients, we ask that you advise us if
you will be late or unable to attend your appointment. If you must cancel, we request at least
24 hours notice so that we may give your appointment time to another patient in need. We have an appointment voicemail
line available to you after hours should you need to notify us of the need to cancel. Patients who
miss more than two appointments without such notice may be asked to leave our practice.
Patient Services
We always have a medical provider available at night and on the weekend. If you have an emergency after
office hours, we ask that you call our office prior to going to the emergency room, when possible.
Our on-call service (nurse and physician) may be able to arrange alternate care that may be less
expensive and involve shorter waiting time. If you have a routine question or problem that can
wait, we ask that you call our office between the hours of 8:00 a.m. and 5:00 p.m. the next
business day.
Our medical providers believe that accurate diagnosis and treatment requires a physical examination
and appropriate diagnostic testing. Therefore, it is our policy that you schedule an appointment to see one of our practioners to treat your medical problem rather than diagnosing or prescribing for your condition over the phone..
If you need a prescription refill, we ask that you contact your pharmacy, and they will call
our office immediately. This process allows our medical providers and pharmacy to review and complete your request
more efficiently. Your pharmacist can ensure that there is no confusion about your medication
strength or directions for refilling.
Our staff is trained to answer many of your questions regarding medical care and consults with
our providers on each call. Our medical providers review their messages throughout the day. For the
quickest response, they may request that our clinical staff return your call. If the medical provider
needs to speak directly with you, they may call at lunch or after hours.
Patient confidentiality is carefully maintained with the utmost respect for your privacy.
Information regarding your medical care will not be released to anyone without signed consent.
In addition to family practice, we also offer services for the treatment of many types of sports medicine injuries and orthopedic problems. These conditions include but are not limited to rotator cuff, AC joint, tennis elbow, bursitis, medical and lateral epicondylitis, carpal tunnel, chronic foot pain, and knee injuries. We are able to see you on the same date you call our office for an appointment to evaluate your condition or injury.
Laboratory Tests
We are able to do basic bloodwork and urine tests in our office. For our patients' convenience,
we can draw blood for most tests, and have the blood sent to our outside laboratory which provides
fast, reliable services.
Our office is able to perform many other tests to assist with the diagnosis and care of our
patients. These include hearing, vision, breathing tests, and electrocardiomylogram.
Hospitalization
If you require hospitalization, we have medical provider partners from our Cornerstone Health Care Group
now available 24 hours per day at High Point Regional Hospital. These "Hospitalists" are
a medical specialty and they provide round the clock hospital care. They will
be in charge of your care in the hospital, coordinate care with any other specialists you
may need, and share with your primary care provider detailed information about your
treatment plan during hospitalization and when you are discharged. We believe that this
team approach provides our patients with the best possible inpatient and outpatient care,
allowing us to be more accessible during office hours for in-office appointments.
Fees and Insurance
At the completion of your visit, you will be advised of the fees for the
day, as well as any prior balance. The cost of your visit is determined by the time we spend
with you, the complexity of your condition, and by the lab tests necessary to properly diagnose
and care for you. In an effort to keep our fees at a just and reasonable level, we request payment
of the portion of the bill that is not covered by your health insurance plan on the date of
your visit with our office.
Your insurance policy is a contract between you and your insurance carrier. It is the responsibility
of you and your insurance company to pay your medical bills. As a courtesy to our patients,
we will file claims to your insurance company, and provide them information to determine benefits.
Your insurance deductible must be met and paid for at the beginning of each year before your
insurance coverage will come into effect.
Our office is a participating provider with most major health insurance plans. Our office staff
will be able to tell you if your plan is on the list of plans accepted by our practice.
If your primary insurance is Medicare, please be aware that some secondary insurance policies
do not cover your deductible. We suggest that you review your insurance policies carefully
to determine what benefits are covered.
