Cornerstone Family Practice at Archdale
Dr. Richard T. Escajeda, Dr. Nick J. Jacobucci, Dr. Sheila Stallings, Physician Assistant Lori L. Beane and
each member of our staff look forward to providing you and your family with quality medical
care in a friendly, comfortable environment.
We are pleased that you have chosen Cornerstone Family Practice at Archdale. Our health care
team is committed to making your experience with our practice as pleasant as possible.
We know that you are very busy and your time is valuable. We believe your time is best spent
sharing your health concerns with our providers and strive to minimize the time you spend in
the waiting room. Please let us know if we are meeting your expectations by sharing your
thoughts with a member of our staff.
Appointments
Scheduling appointments allows us to spend the appropriate time necessary to address your problem. We do, however, hold openings
in our daily schedule so that we may work in patients with acute symptoms or rapidly developing
medical problems. Depending upon the urgency of your condition, we may be able to schedule
your appointment the same day as your call, especially if you phone early in the day.
To help us maintain our timely schedule, and as a courtesy to our other patients, we ask that you
advise us if you will be late or unable to make your appointment. If possible, we request at least
24 hours notice so that we may give your appointment to another patient. Appointments that are
not kept are subject to charge.
Medical Services
As family practitioners, our providers are trained to evaluate and treat a wide range of medical
problems for patients ranging in age from infancy and up.
Among the services we provide are:
- Complete preventive health exams (including school and employment physicals)
- "Sick visits" for cold, flu, urinary tract infections, etc.
- Hospital and emergency care
- Office surgery (vasectomies, mole and cyst removal, laceration repair, skin biopsies, etc.)
- Women's health including Pap Smears
- Screenings for cancer, cholesterol, blood pressure, blood sugar, etc.
- Continuity of care for diabetes, hypertension, coronary disease, respiratory problems,
allergies, arthritis, orthopaedic injuries, etc.
- Onsite laboratory and chest x-ray
If your specific medical condition requires the services of a specialist, we will refer you
to a member of the Cornerstone Health Care team of quality physicians, who will consult with
our medical providers to ensure effective continuity of care during your treatment.
Patient Services
Telephone calls to our office are prioritized in order of medical urgency. If you require
emergency care, please let our nurse know so that we may direct you immediately to our provider, or
the hospital. If you are facing a life-threatening emergency, call 911.
Our physicians believe that accurate diagnosis and treatment requires a physical examination
and appropriate diagnostic testing. Therefore, it is not our policy to diagnose or treat medical
problems over the phone. If you are having a medical problem, we request that you schedule an
appointment to see one of our practitioners.
Our staff is trained to answer many of your questions regarding medical care, and consults with
our providers on each call. Our physicians will return your calls as quickly as possible. If you
need after hours medical assistance, please call our office and our answering service will forward
your calls to the on-call Cornerstone physician who will return your call promptly. Patient confidentiality
is carefully maintained with the utmost respect for your privacy. Information regarding your medical
care will not be released to anyone without signed consent.

Insurance
Our office is a participating provider with most major health insurance plans. Our office staff will
be able to tell you if your plan is on the list of plans accepted by our practice.
Your insurance policy is a contract between you and your insurance carrier. It is the
responsibility of you and your insurance company to pay your medical bills. As a courtesy to
our patients, we will file claims to your insurance company, and provide them information
to determine benefits. You will be asked to pay your portion of the bill at the time service
is rendered. Your insurance deductible must be met and paid for at the beginning of each year
before your insurance coverage will come into effect.
If your primary insurance is Medicare, please be aware that some secondary insurance policies
do not cover your deductible. We suggest that you review your insurance policies carefully to
determine what benefits are covered.
