About Cornerstone Cardiology
Each member of the Cornerstone Cardiology staff looks forward to providing you with quality
cardiac care in a friendly, comfortable environment.
The information presented here is provided to help familiarize you with our practice and policies.

Our health care team is committed to making your experience with our practice as pleasant and convenient
as possible.
Medical Services
The specialists with Cornerstone Cardiology provide comprehensive cardiac care services including:

Your Referral to Cornerstone Cardiology
Once your primary care physician refers you to our practice, we access your medical records
and learn directly from your personal doctor why the referral was made, and the specific details
of your case. When you come for your first visit, we already have some important information
about your medical history and your current symptoms from our communication with your primary
care physician.
We maintain ongoing contact with your primary care physician throughout your treatment plan.
Should you need to be admitted to the Emergency Room at any time, we recommend that you call
your primary care physician. They will alert the Emergency Room Staff that you are coming in
with a cardiac condition and that you should be referred to us. If you don't have time to call
your primary care physician, be sure to let the Emergency Room personnel know that Cornerstone
Cardiology should be called. This will ensure the continuity of care, and allow us to begin your
treatment immediately.
Appointments
We are open Monday through Friday from 8:00 a.m. to 5:00 p.m. If you cannot make your
appointment, we ask that you give us 24 hours notice. Waiting time increases when patients
are late for their appointments, so we ask that you arrive on time for your scheduled
appointment as a courtesy to our other patients.
Please advise our office staff if your address, phone number or health insurance provider
has changed since your last visit. This will enable us to expedite your check-in and assure
that we are able to contact you at your new address or phone should it be necessary to do so.
Please be sure to bring your current insurance information and be prepared to pay for those services
that are not covered by your health plan when you check out with the receptionist.
Patient Services
Telephone calls to our office are prioritized in order of medical emergency. If you require
emergency care, please let our staff know so that we may direct you immediately to our physician,
or the hospital. There is always a doctor available when the office is closed. You may call our regular
office phone number and the answering service will forward your call to our physician. The
physician will determine the appropriate course of action based upon your condition and advise
you immediately. If you have a routine question of problem that can wait, we ask that
you call between 8:00 a.m. and 5:00 p.m. the next office day.
If you need a prescription refill, we ask that you contact your pharmacy, and they will call
our office immediately. This process allows our physician and pharmacy to review and complete your
request more efficiently. Your pharmacist can ensure that there is no confusion about your
medication strength or directions for refilling.
Our staff is trained to answer many of your questions regarding medical care and consults
with our physicians on each call. Our physicians review their messages throughout the day. For
the quickest response, they may request that our clinical staff return your call.
Patient confidentiality is carefully maintained with the utmost respect for your privacy.
Information regarding your medical care will not be released to anyone without signed consent.

Fees and Insurance
At the completion of your visit you will be given an itemized bill showing the fees for the day,
as well as any prior balance. The cost of your visit is determined by the complexity of your
condition, by the tests necessary to properly diagnose and care for you, and the time we spend
with you. In an effort to keep our fees at a just and reasonable level, we request payment of
the portion of the bill that is not covered by your health insurance at this time.
Your insurance policy is a contract between you and your insurance carrier. It is the responsibility
of you and your insurance company to pay your medical bills. As a courtesy to our patients, we will
file claims to your insurance company, and provide them information to determine benefits. Your
insurance deductible must be met and paid for at the beginning of each year before your insurance
coverage will come into effect.
Our office is a participating provider with most major health insurance plans. Our office
staff will be able to tell you if your plan is on the list of plans accepted by our practice.
If your primary insurance is Medicare, please be aware that some secondary insurance policies do not
cover your deductible. We suggest that you review your insurance policies carefully to determine
what benefits are covered.
