About Cornerstone
Cornerstone Health Care is a group of more than 360 physicians and mid-level health professionals representing a wide range of specialties. One of the fastest growing physician groups in the Southeast, Cornerstone is a physician-owned and led multi-disciplinary practice with over 85 locations in communities throughout central North Carolina.
Recognized as a 2012 Success Story Award® winner for patient satisfaction by Press Ganey Associates, Inc., Cornerstone is transitioning from the traditional "fee-for-service" model to a patient-centered medical home system providing expanded access, increased coordination of care, enhanced patient education for prevention and treatment of chronic disease, and sophisticated technological support.
Cornerstone is physician-led and owned. While our physicians have privileges at the many hospitals, surgical centers and nursing homes in the communities we serve, we are not formally affiliated or owned by any of these. Our member physicians are given the opportunity to become shareholders in Cornerstone and they participate in decisions guiding our transition to a paid-for-value future.
Transforming for the Future
Cornerstone Health Care opened in 1995 as a multidisciplinary group of 42 physicians in 15 practices. Today with more than 360 providers in 85 practices, Cornerstone is firmly committed to making the goals of the Three Part Aim the expectations of our patients, payers and employer clients. Since 2011, we have been implementing changes in our organization with three distinct goals: improve the quality of care of our patients; improving the patient experience with all of our providers; and by achieving the first two goals, we will reduce the overall cost of care.
In October 2011 Cornerstone leadership made the decision to move to a value-driven health care delivery system, completely focused on the experience of our patients. During 2012, each Cornerstone primary care practice was recognized by the National Committee for Quality Assurance (NCQA) as a Level 3 Patient-Centered Medical Home™.
In July of 2012, Cornerstone Health Care was selected to participate in the Medicare Shared Savings Program (Shared Savings Program) Accountable Care Organization (ACO) sponsored by the Centers for Medicare and Medicaid Services (CMS). Through the Shared Savings Program, Cornerstone Health Care works with CMS to provide Medicare Fee-for-Service beneficiaries with high quality service and care, while reducing the growth in Medicare expenditures through enhanced care coordination.
As we transition to population health management, we are more closely monitoring the care of patients and working with them on how to properly manage their conditions more effectively.
The Cornerstone Experience
Today Cornerstone patients experience a team approach to their health care that includes the right services, by the right providers at the right time and place. After physician visits, patients receive follow up calls from health care professionals to help individuals work on their goals for improving their health. And Cornerstone asks patients if they are satisfied with their health care experience.
Our clinical information resources allow us to analyze patient care, quality and outcome data and to measure our progress through this transition. Establishing more collaborative partnerships with third-party payers is a critical strategy in our transition to population health management. We have negotiated paid-for-value contracts with all major insurance companies in our coverage area.
